Willamette Dental is an HMO dental plan, which means you must go to Willamette offices to obtain covered treatment. There are more than 20 locations in the Portland Metro area. Highlights of the Willamette HMO Dental Plans include:

MODA Delta Dental PPO
The MODA Delta Dental PPO Plan offers a large network of local and national providers through the Delta Dental PPO network. The highlights of the MODA Delta Dental PPO Plan include:
- $1,250 annual coverage maximum per enrolled individual on the plan
- In-network preventive care does not apply to annual maximum (up to 3 cleanings per year)
- $25 annual deductible for services outside preventive care In-network preventive care covered at 100%
- In-network basic services covered at 80% up to the annual maximum
- In-network major services covered at 50% up to the annual maximum
- Out-of-network coverage but at a lower coverage level
