Vision 

• One annual exam is covered with a $15 co-pay.
• For participants age 19 and above, there is a $200 benefit for frames, lenses, and contacts. It renews every 24 months.
• For participants below age 19, either 1 pair of glasses (lenses and frame) or a 12-month supply of contacts is covered each year.
• A $1,000 lifetime benefit is available for LASIK. 

LAIKAcare offers vision coverage through VSP. This plan provides coverage through both in and out-of network providers. To find a VSP provider, go to: www.vsp.com or call 800-877-7195. VSP does not issue ID cards. Your name and Social Security number are the identification used to access benefits.
